Schedule an annual physical or sick visit with us. Members receive 10% off all a la carte sessions.

Option 2: Membership
We offer a variety of memberships that include Direct Primary Care access.

  • Direct Primary Care puts you at the center of your health. You get personalized, patient-centered care, easier access to your provider, and proactive support for prevention, chronic conditions, and overall wellness. It’s designed to keep you healthy, informed, and supported long-term.

  • You will be seen by Caitlin M. O’Conner, DNP, FNP-C who provides thoughtful, personalized care and guides you in preventive, chronic, and ongoing health management. Learn more about Caitlin here.

  • No. Direct Primary Care is not insurance, does not replace insurance, and does not satisfy any insurance mandate. It is a direct agreement between you and your healthcare provider.

  • Yes. We strongly recommend maintaining health insurance to cover services not included in the DPC membership, such as hospital care, emergency services, imaging, and specialty care.

  • The monthly membership fee for Direct Primary Care is billed directly to the patient and is not eligible for coverage through health insurance. DPC is not health insurance we do not bill or submit claims to insurance for services covered under the DPC membership. For off-site testing and care, you may choose to utilize your health insurance coverage as possible through the outside providers policies. 

    Additionally, Direct Primary Care membership fees are typically eligible for payment using FSA or HSA funds.

  • Your DPC membership may include:

    • Primary care office visits

    • Preventive care and wellness visits

    • Acute care

    • Medication management, excluding controlled substances and higher-acuity mental health medications

    • COVID-19, Influenza A and B, Strep, Blood Glucose, Urinalysis Testing

    • Coordination of lab work completed off-site

    • Chronic condition management

    • Care coordination and medical guidance

    • Direct communication with your provider

  • Services not included in the DPC membership include, but are not limited to:

    • Hospital services or emergency care

    • Specialty care

    • Vaccines 

    • Imaging and laboratory testing

    • Procedures

    • Controlled substances and higher-acuity mental health medications

    • Physical therapy and other ancillary services unless separately agreed

  • No. Membership gives you ongoing access to your provider for visits and follow-ups as needed.

  • We aim to respond within 24-48 hours whenever possible, with slower response times on weekends.

  • Your appointment time is reserved just for you! In order to provide the best care possible, we require 24 hours notice for any cancellations or changes to your appointment. Patients who provide less than 24 hours notice, or miss their appointment, will be charged a cancellation fee to the card on file. The first late cancellation will result in a charge of $50, and the second will be $100.

  • We help coordinate care with outside providers when needed. Services provided by third parties are not covered by the DPC membership and are the patient’s responsibility.

  • Yes. Patients may terminate their Direct Primary Care agreement at any time in accordance with the terms of the patient membership agreement.

  • Physical therapy is offered separately from Direct Primary Care. PT services are not insurance and are not part of the DPC membership.
